CHILDREN S VISION MASSACHUSETTS RESOURCE GUIDE Vision is a critical yet often overlooked component of a child s development physical, emotional, social and educational. One in four American children needs eyeglasses to see the blackboard or to read a book. Parents, physicians, nurses, educators, eye doctors, early intervention specialists and others have a role in assuring that all children have the opportunity to develop their best possible vision. Children who have not experienced clear vision will not know to report a problem seeing. Many young children with a vision problem have no signs or symptoms. All children must have their vision and eye health assessed as part of their newborn physical examination and at every well child visit throughout childhood and adolescence. Children s Vision Massachusetts is an eighty member coalition representing optometry, ophthalmology, nursing, pediatrics, public health, family practice, education, and parents. The coalition s mission is to create a systematic approach to children s vision services in Massachusetts to support early detection, diagnosis and treatment of vision problems in children. With good and timely care, most children s vision conditions respond to treatment. CHILDREN WHO REQUIRE A COMPREHENSIVE EYE EXAM BY AN OPHTHALMOLOGIST OR OPTOMETRIST AS SOON AS POSSIBLE 1. When the child does not pass a vision assessment/screening 2. When parents/others are concerned that the child is having difficulty seeing 3. When the child has signs and symptoms of a vision problem (SEE BELOW) 4. When the child has one or more conditions that puts him/ her at risk of also having a vision problem. These include: Neurodevelopmental Disorders Prematurity and/or Low Birth Weight Motor abnormalities such as Cerebral Palsy Juvenile Arthritis Down Syndrome Cognitive Impairment Hearing Impairment Speech and Language Delays Autism Spectrum Disorders Sibling with diagnosed vision condition as a young child Parent with history of vision problem as a young child High risk maternal behavior during pregnancy SIGNS AND SYMPTOMS of a Vision Problem Eyelids are red-rimmed, crusted or swollen Eyes are watery or red (inflamed) Eyes don t line up, one eye appears crossed or looks out An eyelid that droops, covering the eye even when open Rub his/her eyes a lot Tilt his/her head or thrust head forward Hold objects close to his/her eyes Blink more than usual Squint or frown Not maintain eye contact when awake and alert Common Eye Conditions In Children Many young children with a vision problem have no signs or symptoms. Formal assessments of vision are important throughout a child s life. Lazy Eye/Amblyopia is a loss of vision that is not corrected by glasses. Eyes that don t line up, or having one eye that focuses better than the other, can cause amblyopia. Strabismus is a word for eyes that are not straight or do not line up with each other. If the problem is not treated, it can cause amblyopia. Refractive Errors may create focusing problems at far, near or both and may also cause amblyopia. Refractive error conditions include hyperopia (far-sightedness), myopia(near-sightedness) and astigmatism. FREQUENTLY ASKED QUESTIONS What is the difference between a vision screening and a comprehensive eye exam? Vision screening identifies children at risk for vision problems and may result in a referral to a comprehensive eye exam. A comprehensive eye exam is performed by an ophthalmologist or optometrist and may result in a diagnosis of a vision condition and treatment plan. Who should perform a comprehensive eye exam on my child? There are two types of eye doctors optometrists and ophthalmologists. An optometrist is an eye specialist with a Doctor of Optometry (OD) degree. Optometrists perform comprehensive eye exams, diagnose eye problems, prescribe glasses, and if surgery or other complex care is needed, optometrists refer patients to ophthalmologists. An ophthalmologist is an eye specialist with a Doctor of Medicine (MD) degree. Ophthalmologists perform eye exams, diagnose vision problems, prescribe glasses, and perform eye surgery. How old does my child have to be to have a comprehensive eye exam? Children can receive a comprehensive eye exam at birth. How often should my child be seen by an eye doctor? This is different for every child. If your child is referred for a eye exam, the child s eye doctor will decide how often appointments should be scheduled based on the results of the exam. What is an optician? When a child is prescribed eyeglasses, s/he will see an optician. Opticians fit, dispense, and adjust eyeglasses. How should eyeglasses be selected for my child? If children are old enough, they should be allowed to choose their own frames as they are more likely to wear glasses that they pick out themselves. Plastic frames are best for young children. What if my child s insurance doesn t cover eyeglasses and my family cannot afford to buy them? The family should discuss this with their child s Early Intervention Specialist, Health Manager, Pediatrician or Family Physician to ask about assistance programs. When a young child is prescribed eyeglasses it is very important that they wear them. What if my child loses or breaks his/her eyeglasses? The family should discuss this with the eye doctor or contact the eyeglass store, eye doctor office or health center where the child got his/her most recent pair of eyeglasses and request a new pair. Many insurance plans, including Mass Health, will replace lost or broken glasses. Will my child outgrow the need for eyeglasses? This is different for every child; it is a question that the child s eye doctor will be able to answer. If one of my children wears eyeglasses, will my other children need them? Vision problems are common in children and do run in families. When one child wears eyeglasses there is a greater chance that siblings will also need them. INSURANCE BENEFITS Families should confirm that the child s insurance will be accepted by the eye doctor before the eye doctor appointment. Families should confirm before ordering eyeglasses that the child s insurance will be accepted at the eyeglass store. Prescriptions for children s eyeglasses are good for 12 months; frames should be selected and eyeglasses ordered as soon as possible after being prescribed. Families should keep the original prescription and ask the optician to make a copy for their records in case the eyeglasses are lost or broken. One comprehensive eye exam, by an eye doctor, each year When ordered by eye doctor, additional eye exams during same year Eyeglasses ordered by the eye doctor Replacement eyeglasses for lost or broken glasses NO LIMIT on number of replacement eyeglasses When ordering eyeglasses: It may take 1-2 months for the eyeglasses to be ready so the eyeglasses should be ordered as soon as possible. The child/ family must return to the eyeglasses store that the eyeglasses were ordered to pick them up so that the optician can check for fit, and make sure the lenses are correct. Policies and procedures in eyeglass stores are established by Mass Health and do not differ between stores. To re-order eyeglasses that have been lost or broken: Child, parent, health manager, school nurse or other advocate, should call the store where the lost/broken eyeglasses were obtained and report that the glasses are lost or broken and ask that another pair be ordered. OR, take the prescription to an eyeglass store that accepts Mass Health insurance and reorder the eyeglasses. If it has been less than one year since the first pair was ordered, the same frame choice as lost or broken eyeglasses will be used. If it has been more than one year since the first pair was ordered, the child should have a comprehensive eye exam before re-ordering the eyeglasses to be sure the prescription is correct. PRIVATE INSURANCE If a child has private insurance, eyeglasses may not be covered completely, or at all, and the family will be asked to pay full cost or a copayment. ASSISTANCE When eyeglasses are not covered by insurance, and financial qualifications are met, assistance for eyeglasses is available by contacting Priscilla Kelangi, at Prevent Blindness.
